Diabetes Care Clinic has the ability to download data from personal blood glucose monitoring devices, continuous glucose monitors, and insulin pumps at each visit. We can also remotely retrieve data if available based on the system used to assist with treatment decisions.
Point of Care Labs
We offer the convenience of an in-house lab to assess A1c, lipid, and kidney function tests that produce immediate lab results. With timely results we can optimize the effectiveness of current therapy by making adjustments to the patients plan of care.
Continuous Glucose Monitoring
CGM is a small, waterproof device that is worn by the patient for several days. This device automatically tracks glucose levels day and night. CGM can provide a complete picture of trends and patterns. This information is helpful in determining the best treatment to better manage diabetes.
Patient Education
We provide education and support regarding multiple topics related to diabetes care. Our goal is to equip and empower patients with the necessary knowledge and skill to successfully managing their diabetes and prevent complications.
Patient Portal
Our patient portal is a secure online website that gives patients convenient, 24-hour access to personal health information from anywhere with an Internet connection. The portal allows patients to interact and communicate with the diabetes team and receive support between visits.
